EQUIPP® Account and Log In Information

  1. How to obtain EQUIPP® Access:
  • We maintain contractual agreements with various Pharmacy Services Administration Organizations (PSAO) and third-party contracting entities. In certain instances, pharmacies may be required to register through their designated group.
  • Pharmacies that are not associated with an organization can gain access through an independent contract by reaching out to EQUIPP® Support at support@pharmacyquality.com.
  1. How to log into EQUIPP®:
  • Access EQUIPP® in two ways:
  • Log in directly at  EQUIPP.pharmacyquality.com . After account creation, users receive an email with login instructions, including a link to set their password and Multi-Factor Authentication (MFA). For MFA guidance, see  here . If you haven't received a welcome email, click "forgot password" on the login page. If your username is valid, you'll get a password reset email from  support@pharmacyquality.com . If you don't receive an email, contact us at  support@pharmacyquality.com  for help.

  • Single Sign On (SSO) Login – Some pharmacies log in via their site using SSO, eliminating the need for a separate EQUIPP password. If your organization uses SSO, log in through their channels and contact EQUIPP® Support for assistance if needed. How to update account information:
  • Name Updates: End users can update their first and last names in the Settings tab, accessible by clicking the orange circle icon at the top right of the Performance Dashboard. (See image below)

  • Click 'Settings'

  • A pop-up window will display with the option to enter the new First and Last Names
  • Click 'Confirm'
  • This will save your changes and display them within the EQUIPP Dashboard
  • To add or remove pharmacies from your EQUIPP Dashboard: Pharmacies should make all requests to add or remove locations through their PSAO.
  • To update usernames or add additional users: Pharmacies will need to place all account updates and new user requests through their PSAO
  1. How to reset your password:
  • End users that log in directly into the EQUIPP® site should set their password upon first login with the "Set a password" link provided in their welcome email. After the initial login, end users will be able to reset their password with the “forgot password” link.
  1. Seeing an error when logging into EQUIPP®? (Error message: “Getting Data” loading screen)
  • Try these steps:
  • Clear browser cache and cookies
  • Use a different browser
  • Use a different device
  • Disable your ad-blocker (if applicable)

To reset or change the phone number for MFA, follow these steps:

  1. Log in, then click the orange circle in the top right of your EQUIPP Dashboard.
  2. Click 'Settings'.
  • A pop-up will appear to change your name or 'Reset MFA'.
  1. Click 'Reset MFA'.
  • A confirmation pop-up will appear, logging you out and allowing you to set a new phone number for MFA.
