The Hemoglobin A1C Data Gap Closure Program (A1C) is designed to empower pharmacy staff members, including pharmacists, nurses, and nationally certified technicians (CPhT) to help fill data gaps when a patient does not have an A1C reading on file with their health plan for the current year.
