Longitudinal Adherence Monitoring Program (LAMP)

The Longitudinal Adherence Monitoring Program (LAMP) is designed to improve and maintain medication adherence for targeted patients while rewarding high-performing pharmacies for documented interventions and patients that are adherent (Proportion of Days Covered or “PDC” of 80% or better) at the end of the calendar year. The program will evaluate the following core adherence measures

  • Diabetes PDC
  • RASA PDC
  • Cholesterol PDC

Additionally pharmacies have the opportunity to receive payment for closing Statin gaps in care for:

  • Diabetes (SUPD)
  • Cardiovascular Patients (SPC)

Patient adherence scores and newly eligible patients will be updated every Friday in the Enhanced Services Tab within the EQUIPP Performance Dashboard (under "Longitudinal Adherence Monitoring Program"). Ensure that you track your patient's adherence closely throughout the year to unlock additional revenue opportunities!

The final program analysis will examine the Year-to-Date (YTD) measurement period from January 1 - December 31, 2025.

Longitudinal Adherence Monitoring Program (LAMP)

Payment in this program will be based upon completing encounters with targeted patients provided by the health plan via EQUIPP. Types of encounters include quarterly check-ins, extended days supply conversion, and final fill interventions.

  • Quarterly Check-Ins: XX per check-in per each patient/measure opportunity (up to 3 check-ins per each patient/measure opportunity year). Check-in dates are:
  • Check-In 1: April - June 2025
  • Check-In 2: July - September 2025
  • Check-In 3: October - December 2025
  • Extended Days Supply (EDS) Conversion: XX per validated conversion (up to one conversion per patient/measure per year)
  • Final Fill: XX per validated conversion
  • The opportunity provided select patients eligible to reach adherence for 2025 by receiving one last fill for the year
  • Available approximately in late September through December 2025
  • Expect SUPD and SPC opportunities to begin later this year.

All above encounters must be documented via EQUIPP Enhanced Services to be eligible for payment. NOTE: Not all patients will be eligible for all types of encounters.

1. Navigate to the Enhanced Services Page WEEKLY!

  • Targeted patients for LAMP will be displayed in the Enhanced Services Tab on the left hand side of the screen of your EQUIPP Performance Dashboard within the program labeled "Longitudinal Adherence Monitoring Program"

2. Document!

  • Both active and previously documented opportunities will be listed under the patient. Opportunities that have expired without documentation will not be listed
  • To document an intervention, click on "Not Started" in the documentation status column:

  • The documentation window will then pop up. Complete the applicable questions after speaking with the patient. Please note that the documentation will apply to all currently available intervention opportunities

  • If the intervention requires validation, fill the prescription after documenting the intervention. Prescriptions must be filled on or before the "Expiration Date"

3. Keep Your Patients Adherent!

  • Patients' PDC rates will be updated weekly starting April 2025 to allow you to track your patients' adherence throughout the year
