Controlling Blood Pressure (CBP) Program

The Controlling Blood Pressure (CBP) gap closure program is designed to empower pharmacy staff members, including pharmacists, nurses, and nationally certified technicians (CPhT) to help fill data gaps when a patient does not have a blood pressure reading on file with their health plan for the current year.
We’ve created some resources that will assist your pharmacy as you record readings and provide advisement to your patients which include:
